PURPOSE:   

A key member of the Rehab Services professional staff responsible for clinically competent evaluations and treatment programs for all assigned patients along with documenting care according to departmental standards.  Represents Sherman Health within the community.

ACCOUNTABILITIES AND JOB ACTIVITIES:

Performs patient care services:

  • Demonstrates knowledge of injury, illness and/or disability in populations serviced 
  • Demonstrate clinical competency and performance in the following areas: assessment tools, intervention programs, exercise equipment, modality equipment, measuring and monitoring devices, home program development, computer skills, communication skills, interpersonal skills with patients, family, staff and others 
  • Demonstrate and promote team work with flexibility, willingness to assist other departmental service locations as requested, showing respect and cooperation with all departments and individuals, living the standards or behavior
  • Provide professional education to students, staff and the public
  • Responsible to fulfill all requirements of the Physical Therapy job description
  • Functions as an expert clinical resource for populations served
  • Facilitates interdisciplinary teamwork to ensure patient care needs are met in an efficient and effective manner
  • Implements and monitors plan for care to ensure timely outcome achievement
  • Provides education to team members, caregivers, and patient to ensure achievement of treatment plan goals

SKILLS:

  • Basic computer skills
  • Satisfactory completion of core competencies
  • Ability to integrate elements of patient/client management-examination, evaluation, diagnosis, prognosis, and intervention - in a manner designed to maximize functional outcomes
  • Ability to push-pull, lift-carry, and transfer patients

EDUCATION AND EXPERIENCE:

  • BA or BS in Physical Therapy

CERTIFICATION:

  • Current IL state license
  • Current CPR

Weekend and/or Holiday Rotation may be required.
