Job Summary:

Provides nurse aide services in accordance with policies, procedures and guidelines of Sherman Home Care Partners.

Job Responsibilities:

  • Demonstrates excellence in customer satisfaction.
  • Follows a plan of care. Effectively communicates with the clinet and other employees taking care of the same patient.
  • Provides and documents clinical care to completely and accurately reflect the quality of care.
  • Completes and submits progress notes and billing tickets on time.
  • Supports decision making.
  • Compliance with regulatory and JCAHO requirements.

Education:

Graduate from accredited nurse aide program.

Certification:

  • Current IL Nurse Aide certification
  • Current CPR
