PURPOSE:
The Patient Care Coordinator (PCC) has the responsibility, authority, and accountability to facilitate and deliver patient care in collaboration with the staff registered nurse, case management, and manager. The PCC utilizes the nursing process and critical thinking skills to direct and deliver comprehensive nursing care to patients on an assigned unit in a safe and proficient manner according to individual patient needs. Provides direct patient care based upon the standards set within Illinois Nurse Practice Act as well as organization specific policies and procedures. The PCC coordinates the ongoing assessment, planning, intervention, and evaluation for patient care for an assigned patient population and ensures patient safety and satisfaction.

EDUCATION AND EXPERIENCE
Must have valid State RN License
Bachelor of Science degree in Nursing or health related field preferred, or specialty certification
Must have at least 5 years of patient care, 3 of which being in home health
Must have at least 2 years of supervisory experience
CERTIFICATION
BLS Certification
Current Illinois Registered Nursing License
